To use XPortal you must first create a User Name and Password. Enter a User Name and Password that you will remember.

The User Name must be unique. If the User Name is already taken, you will get an error message.

The password must be at least seven characters with at least one letter, one number and one special character. Valid passwords are: Bob123!, 1Jones!, $tiger45.

Make sure that your email address is correct. Your email address will only be used to recover lost passwords. Your email address will not be shared with any person or organization for solicitation.

Enter a security question that only you know the answer to. This question will confirm your identity for password recovery.

After you click the Create User button, please fill out the online survey. All new users must complete a survey before the account is activated. After your account is activated, you may create forms online.
